2026 Client & Patient Information Update Registration

The Client and Patient Information Update Form is required for all existing clients as part of our 2025–2026 transition to a new medical records system. This form ensures that all owner information, co-owner details, contact numbers, mailing addresses, and patient records are fully updated and verified within our new database. Accurate and complete information allows us to maintain current medical records and support uninterrupted communication and care.

Update Your Client Information

The Client Contact and Emergency Information Update Form is used when clients need to provide revised phone numbers, email addresses, mailing addresses, or changes to authorized emergency contacts. Maintaining updated contact information allows our staff to communicate efficiently regarding medical care, scheduling, and urgent situations.


Co-Owner Registration Form

The Co-Owner Registration Form is reserved for adding an additional owner or responsible party to a pet’s permanent medical record. Only individuals designated as owners or co-owners are permitted to make medical and financial decisions on behalf of the pet. This form ensures clarity regarding authorized decision-makers and supports consistent, accurate documentation.

The Virginia State Legal Disclosure and Consent Forms serve to meet all regulatory requirements mandated for veterinary practices in the Commonwealth of Virginia. These documents provide required notifications and obtain legally necessary permissions related to treatment, patient handling, and communication. Every client must have complete and current disclosure forms on file.
